bottom line - everyone at the refinery was clueless - $500 million
loss - they did not know how to do a safe shutdown - shutdown plans
had not been analyzed by offsite experts for 60 years - no one put
foam on huge asphalt leak for 2 hours - etc
seems they came close to killing 10,000 with poison gas
when humans are using complex procedures - and something goes wrong -
