patients - this is my interpretation
they billed for $40M but only got $20M (per pg8 of 20) - so it looks
like they are billing for the right amount but the insurers are giving
them only 50% of what they ask for

most ambulance crews / firefighters make $30 per hour approx - 1 amb
trip takes 1 hour and takes 6 people - so personnel costs are around
$180 - and the county bills either $750 or $1200
crazy numbers all around
