level 1 - inhouse general surgeon plus a bunch of oncall surgeons -
plus a teaching program
level 2 - "immediate" coverage by general surgeons and some oncall
surgeons - this almost sounds like general surgeon does not have to be
inhouse
level 3 - maybe an emergency room doc backed up by an oncall general surgeon
maybe a level 2 could have a inhouse general surgeon daytimes and an
oncall general surgeon overnight - or maybe vice versa
or maybe a level 2 is the same as a level 1 but there are just fewer
specialized surgeons oncall
