ADVANCED LIFE SUPPORT OR ALS INTERCEPT 1. When indicated in protocol,
call for ALS intercept. If ALS intercept is unavailable, or the
transport time to the closest appropriate facility is less than the
ALS intercept, initiate transport to the closest most appropriate
facility
from https://www.nj.gov/health/ems/documents/NEWJERSEYEMSCLINICALPRACTICEPROTOCOLS_GUIDELINES_FINAL8.21.2025v1.pdf
if the hospital is 20 minutes away - and the als is coming from the
hospital - then the guideline demands that the ambulance sit at the
scene and wait for the als to arrive
because the intercept time will be the same as the transport time
but its weird - because the section is labeled as INTERCEPT
common sense says that the patient and the als will meet halfway to
the hospital - which means the patient will receive als in 10 minutes
versus 20 minutes
what if the guideline said 'less than or equal to' ???
this is probably some sort of a gaming problem that a statician could
solve - common sense says that the goal is to get als for the patient
as quickly as possible - but the transport time to the hospital should
be as low as possible also
question - do you want to drive the patient away from the hospital to
reach the als quicker? probably depends on the specific problem - a
blocked airway requires immediate als because the consequences are
100% - death in 5 minutes
